2023 Announcement for Medicare - Return to Fee for Service and Chronic Care Management

Progressive Health is committed to using whatever innovations we can to serve our seniors best, WITHOUT charging any concierge fee.  As we continue to evaluate the best way to take care of our patients, we are making a change for 2023.  In 2023, we return to the Medicare Shared Savings Program and fee-for-service, with Chronic Care Management (CCM) as our preferred way to deliver the high value care that we are all accustomed to.  

This announcement ONLY applies to people receiving traditional Medicare. For all other insurances, nothing is changing.

As a Medicare recipient, you will see very few changes.  We are following the same billing as we have for the past 20 years with the exception of 2022. 

Progressive Health is proud to employ a team of highly skilled Registered Nurses, Care Navigators, and Health Coaches, devoted mostly to the health of our seniors and our Chronic Care Management (CCM) Program.  This team provides the personalized level of care that is required to ensure every need of our patients is met.  Our care team will continue to deliver these services to patients who are currently enrolled in Chronic Care Management.   For most Medicare patients, there is no cost to you for participating in Chronic Care Management.  For some patients, there will be a copayment each month that the services are delivered, typically around $9.  

We believe Chronic Care Management is a vital service that is required for patients to receive the care they need and expect, and helps us to keep our patients healthy.
